In order to get set up for our dispatch services, please complete the Application form below. Once received we will set you up in our system and schedule your truck for dispatch!

• Fill out a basic form with your Name, Email, and Callback Number

• Upload your FMCSA Operating Authority (MC Certificate)

• Submit the W-9 Form (Request for Taxpayer Identification Number and Certification)

• Provide your Certificate of Insurance, including liability and cargo policies

• Review and sign the dispatch agreement
